VAN WERT - The Van Wert Post of the Ohio State Highway Patrol is currently investigating a fatal motor vehicle crash that occurred on Thursday, September 30 at approximately 3:58 p.m. The single vehicle crash occurred on State Route 114, west of Grover Hill.

The driver of the 1995 GMC Safari van was S. Jean Dyer, 64, of Paulding. Dyer was pronounced dead at the scene by Dr. Kuhn, Paulding County Coroner.

The 1995 GMC was westbound on St. Rt. 114 when it drove off of the north side of the roadway and struck a tree. There were no other vehicles involved, and the cause of the crash remains under investigation.

Assisting at the scene were the Paulding County Sheriffs Department, Grover Hill EMS and Fire Department, Samaritan Life Flight and Scott EMS.
